Childhood Adversities and Psychosis Across Populations: Insights From the 6-Country EU-GEI Study

Abstract Background and hypothesis: Using data from the EU-GEI Work Package 2 (EU-GEI WP2) programme, we sought to test several hypotheses related to gaps in our knowledge of associations between childhood adversities and psychosis. Study design: EU-GEI WP2 comprises incidence and case-control studies of first-episode psychosis conducted in 17 sites in 6 countries. In each site, over 2-year periods, we identified and collected relevant data from individuals aged 18-64 with a first-episode psychosis and with no history of psychosis. Missing data were imputed. We used multi-level logistic regression to test our hypotheses. Study results: In total, 1071 cases and 1497 controls were included. We found variations in the prevalence and the magnitude of associations between any adversity and psychosis by place (eg, odds ratios ranged from 0.4 [Cuenca, Spain] to 12.1 [Madrid, Spain]). The weighted percentages reporting adversities in control samples were associated with site incidence rates (eg, 3+ adversities: Spearman's rho 0.56, P .025). We found variations in the magnitude of associations by sex (eg, effect of physical and sexual abuse stronger among women), by age of exposure, and by severity and frequency of adversities (eg, largest odds ratios for adversities involving hostility, threat, and violence). Conclusions: Variations across populations in prevalence and effects of adversities may contribute to variations in rates of psychosis. Variations in effects by sex and age of onset may point to sex-specific mechanisms and to developmentally sensitive periods. Adversities involving severe threat, hostility, and violence may have the largest effects on risk of psychosis.

Family functioning, attachment security, and psychological stress among university students attending a psychological counselling service: an exploratory cluster analysis

Abstract Background: Psychological stress is highly prevalent during higher education studies but mental health trajectories are shaped by risk and protective factors. Attachment security and balanced family functioning might reduce students' psychological stress but literature is still limited. The aim of this study was to investigate the relationship between psychological stress, family functioning, and adult attachment in order to identify more vulnerable groups. Method: A sample of university students self-referring to an on-campus counselling service (n = 171; 81.9% female and 18.1% male; Mean age = 22.53 years, SD = 3.35) was assessed for psychological stress, family functioning, and adult attachment. A Two-Step Cluster Analysis and ANOVA were conducted to identify cluster groups reflecting distinct profiles and to test between-group differences. Results: Sixty-three percent (63.2%) scored above the CORE-OM clinical cut-off. Distress correlated positively with unbalanced family patterns (Disengaged, Enmeshed, Rigid, Chaotic) and with Attachment Anxiety and Avoidance; and negatively with Cohesion, Flexibility, Satisfaction, and Communication. A two-cluster solution emerged: (1) High Distress / Low Relational Functioning with lower balanced functioning, higher unbalanced patterns, and higher attachment insecurity; and (2) Low Distress / High Relational Functioning with the complementary pattern. Conclusions: Student distress was systematically intertwined with family functioning and attachment, and coalesced into two clinically meaningful profiles. Findings support mechanism-focused, stepped care in campus services: low-distress students may benefit from low-intensity psychoeducation and skills rehearsal, whereas high-distress students may require brief, structured modules targeting communication, boundary-setting, and emotion regulation, with referral pathways activated when complexity or acute risk is present.

When the body speaks of loss: psychosomatic dysregulation as a mediator between insecure attachment and prolonged grief symptoms in oncological and traumatic bereavement

Abstract Background: This study investigated the role of psychosomatic dysregulation as a mediating factor in the association between attachment insecurity and prolonged grief symptoms. Methods: A cross-sectional design was used to survey 227 adults who had lost a loved one due to oncological (n = 138) or traumatic causes (n = 89) (74.0% female; age = 48.11 ± 11.53 years). The two groups did not differ significantly in terms of age, gender, marital status, or living arrangement. Participants completed the relationship questionnaire, the psychosomatic dysregulation inventory, and the traumatic grief inventory SR +. Results: The severity of prolonged grief symptoms did not significantly differ between groups, although participants in the oncological loss group reported slightly higher symptom severity compared to the traumatic loss group. The prevalence of probable PGD was similar between groups, with 13.8% of the oncological group and 10.1% of the traumatic group meeting diagnostic criteria. Results of mediation analysis showed that psychosomatic dysregulation partially mediated the relationship between attachment anxiety and prolonged grief symptoms, whereas no significant mediation was observed for attachment avoidance. Discussion: These findings point out the importance of addressing psychosomatic processes in individuals with high attachment anxiety, in line with the hyperactivation model of the anxious attachment system, to better understand and support their grief responses.
